Overview
The aim of this document is to support patients' choices to avoid long stays in hopital. It applies to all adult inpatients in Norfolk & Waveney NHS settings and should be utilised before and during admission to ensure that those who are assessed as medically optimised for discharge can leave hospital in a safe and timely way.
