
Complex Transition information evening – 30 April 2025
We are holding a transition information event to support young people with complex health and learning needs in their transition from paediatric to adult care.
The event will be held on the Wednesday 30 April from 10.30am to 1.30pm. The event is aimed at young people 13 years + with complex health and learning needs under any speciality at NNUH who are transitioning from children to adult services in the future. Parent and carers are welcome.
Transition information will be given as well as talks on:
- The role of our Learning Disability and Autism Team
- The Mental Capacity Act, power or attorney and deputyships
- Advanced care planning
- What to expect in adult healthcare
There will be representatives from children and adult services such as Continuing Care, PFAL (preparing for adult life), Epilepsy nurses and the Home Enteral Feeding Team available to talk with families outside the lecture theatre.
Sarah Cassels, Lead Transition Practitioner, said: “Moving from paediatrics to adult service can often be a daunting time for young people and their families and carers. Transition aims to gradually address the move over a period of time and introduce young people and their parents or carers to the adults teams involved in their care before they leave paediatrics. This ensures there is the opportunity to ask questions and promotes young peoples’ independence in their health care.”
The event is taking place in the Benjamin Gooch Lecture Theatre, East Atrium, Level 1. Refreshments will be provided and parking in our surface car parks will be free for attendees.
