Palliative Care
We offer specialist advice and support both on symptom control and the total care of patients with any life-limiting illness. We provide patient centred care and care for the family and carers. We also support healthcare professionals involved in the care of patients, families and carers who are receiving Palliative Care.
We are an advisory service, which works in partnership with the referring team. We see patients from any department and offer both in-patient and out-patient care, included a dedicated breathlessness clinic.
We work in partnership with the patient’s medical team and can be involved at any stage in the patient’s illness from the time of diagnosis onwards. Our involvement is targeted at patient and family centred care, concentrating on working with the patient to achieve the very best quality of life and to support them and their family.
Our team of Doctors and Nurses provide face to face assessments, 7 days a week including Bank Holidays.
