Hearing aid accessories request form

We are pleased to offer you the option to request hearing aid accessories online for free delivery to your home address.  This includes items like batteries and tubing.

Please complete the form below then click ‘submit’. Your request will be sent to our team who will look up your records to confirm the exact type or size of parts needed.  We aim to post accessories out to you within one week.

PLEASE NOTE:

Only patients fitted with hearing aids by the Norfolk and Norwich University Hospitals Trust are eligible for this service.

We will only send consumables when they are due (every four to six months for tubes and every year for batteries). If we receive a request before this, we may need to contact you to discuss your request further.
