Young patients and carers invited to transition information evening
An information evening is being held in October for young people and their carers to discuss moving from the care of Children’s Services to Adult Services at the Norfolk and Norwich University Hospitals NHS Foundation Trust (NNUH).
It follows last year’s successful transition evening where members of the public were able to meet the teams that would be caring for them in the future.
The event will be held on Monday 7 October from 5:30pm – 7:30pm in the Benjamin Gooch Lecture Theatre. The event is aimed at young people under all specialities aged 13 and above, with long term conditions, who will be transitioning from paediatric to adult services.
Transition information will be given as well as general adolescent health and wellbeing support, including talks on consent and confidentiality, and managing your own health. There will also be information about the Norfolk and Norwich Youth Forum, a group set up to help improve patient experience for young patients.
There will be representatives from all adult and children’s services with stands for people to talk to members from both teams providing personalised and speciality specific information, as well as tours of some adult out-patients areas.
Sarah Cassels, Lead Transition Practitioner (pictured), said: “These evenings are an opportunity to allow patients to meet the adult teams for the first time in a relaxed environment. They also allow us to give age-appropriate information to young people so that they are empowered to manage their long-term health conditions as they become young adults.
“We’ll also have some fantastic members of our Youth Forum who’ll be sharing how they’re helping to give us feedback and make improvements to patient experience. They’ll also be recruiting for new members. We hope to see as many patients, families and carers there as possible.”
The event is taking place in the Benjamin Good Lecture Theatre, East Atrium, Level 1. Refreshments will be provided. Parking will be free for attendees.
