Overview
The Individual Plan of Care for End of Life EOL will replace the patient care record and Nursing Assessments and Plans of Care documents following recognition that the patient is entering the last hours/days of life and this has been communicated with the patient and/or family/next of kin as appropriate. This is a multi-disciplinary record and should be completed by the medical nursing teams and any other health or social care practitioner who is involved with the patient.
