Under The Data Protection Legislation, you have the right to request a copy of what information we hold about you.  If you would like to request a copy of your information, please visit the Health Records page on this website.

In most circumstances we will not charge you for requesting your information.  We will respond to your request without any undue delay and within a month after we receive your request.
