Pre-screening

Pre-screening is the first step in the pre-operative assessment process.

Within one week of your appointment with the clinician, where it is decided that you will undergo a surgical procedure, you will receive a personalised link to a pre-screening questionnaire from a company called DrDoctor via text message and/or email, depending on the contact details we hold for you.

DrDoctor will ask questions about you, your current health and your medical history. The questionnaire should take around 20-30 minutes to complete but may take longer depending on your individual circumstances and medical history. You can save your progress as you go and return to the questionnaire later if needed. However, it is worth noting that it is important to complete this questionnaire as soon as you can as we often  assign dates for operations once the pre-assessment has been completed, so any delay in completing this questionnaire, may result in a delay to your surgery.

To log into the questionnaire, you will need to click on the link and then enter your Surname, date of birth and postcode. There is also an option to log in using your NHS account username and password if you have these set up.

If you are unable to complete the questionnaire online, you have a few options:

  1. If you are not comfortable completing this questionnaire on your own, or do not have the facilities to access the weblink at home, it is fine to ask a family member or friend to help you.
  1. Alternatively, please feel free to contact the POA Patient Flow Coordinator on 01603 647923 who will be able to arrange for someone to give you a call to help you complete the questionnaire.
